Corporate Resolution - This documentation is required if your company entity is one of the following: LLC, LLP, Partnership, or Corporation.

Email / Fax Authorization - Review and execute

W-9 Form - Request for Taxpayer Identification Number and Certification.

Broker To Provide

Broker License(s) - Attach the broker license(s) in the State(s) you are currently approved to do business in.

Resumes - For all owners, principals, officers, and broker of record.

Current Financials - Including, but not limited to Balance Sheet an Income Statements. All financial information provided will be held in strictest confidence and will not be released to any outside party.

NOTE: The approval process will begin once we have received a completed broker package. Security Mortgage Funding is unable to fund your loan until your broker application is approved.
